Health Care Directives, Living Will, & Power of Attorney forms:

In the State of Arizona, you may use any Health Care Directive you choose. However, Notaries or Notary Signing Agencies may not offer, sell, or provide you with these forms. Our State does provide an excellent package of forms if you wish to review at: ARIZONA LIFE CARE PLANNING PACKAGE

Once you have chosen the form you wish to proceed with you may schedule an appointment with one of our Notaries. Please note: many Life Planning forms require an additional witness (or two) in addition to the Notary Public's Notarization. Notaries are not allowed to provide a witness. Please inspect your document prior to scheduling an appointment for the Notary to make sure you can arrange for a witness to be present if needed. 

 

Pricing for Arizona:

We charge $15 admin, $50 Travel & Time,
and $10 per notarized signature. We offer bookings within 2 hours' notice.
